Neonatal sepsis

 

Neonatal sepsis is a blood infection that occurs in an infant younger than 90 days old. It is a leading cause of morbidity and mortality among infants, especially in middle and lower-income countries. Neonatal sepsis is divided into two groups based on the time of presentation after birth: early-onset sepsis (EOS) and late-onset sepsis (LOS) . Early-onset neonatal sepsis most often appears within 24 to 48 hours of birth, and the baby gets the infection from the mother before or during delivery. Late-onset neonatal sepsis occurs after 1 week through 3 months of age and is usually acquired after delivery. Neonatal sepsis can be caused by bacteria such as Escherichia coli (E. coli), Listeria, and some strains of streptococcus. Group B streptococcus (GBS) has been a major cause of neonatal sepsis, but this problem has become less common because women are screened for GBS during pregnancy. Neonatal sepsis can affect any baby, but premature babies develop sepsis more often than full-term babies because their immune systems are immature. The most common early signs of sepsis in infants are fever or hypothermia, tachypnea, lethargy, or new parental reports of poor feeding. Infants with neonatal sepsis may have multiple nonspecific symptoms, including diminished spontaneous activity, less vigorous sucking, swollen belly, cold hands and feet, clammy, pale skin, yellow skin and whites of their eyes (jaundice), reduced activity, and seizures.

 

Prevention of neonatal sepsis

Neonatal sepsis is a serious medical condition that affects babies younger than 28 days old. It is a leading cause of morbidity and mortality among infants, especially in middle and lower-income countries. Preventing neonatal sepsis requires a multidisciplinary approach, including hygienic practices, appropriate cord care, early initiation and exclusive breastfeeding, maintenance of warmth, and prompt treatment with antibiotics. Here are some strategies to prevent neonatal sepsis:

 

– Reduction of the exposition of newborn infants to pathogens: hand hygiene practices; proper management of central lines; promotion of early enteral feeding with human milk; prophylaxis with lactoferrin and fluconazole.

– Improvement of neonatal defenses: lactoferrin and human milk.

– Identification of high-risk infants: this is a central point including, but not limited to, the use of metabolomics for risk stratification.

– Application of 7.1% chlorhexidine on the umbilical cord stump (compared to the previous practice of keeping the cord clean and dry) has also proved to reduce newborn mortality.

– Maternal immunization practices to prevent both late-onset infection and early-onset infection and/or infection-related preterm birth.

– Early initiation of breastfeeding, within an hour after birth, would further reduce newborn deaths—partly as a result of prevention of newborn infection through the provision of antibodies to assist the baby fight infection.

 

To prevent passing along an infection to your baby, your obstetrician may recommend you take preventive antibiotics.
